Living Better
What is it?
The West Somerset Living Better service is a multi-agency approach to providing personalised and co-ordinated care for patients to support independent living.
Who is it for?
Living Better is a service available to all patients registered with any of the practices within the West Somerset Primary Care Network (Williton and Watchet Surgeries, Dunster and Porlock Surgeries, Minehead Medical Centre and Exmoor Medical Centre).
What will it do?
The GP practices in West Somerset are each supported by attached Living Better nurses and Village Agents. The Living Better team work closely with GPs and other agencies involved in a patient's care (for example social services, hospitals, community services, voluntary grous, etc.) to provide a co-ordinated and holistic approach to personalised care. By combining healthcare with social and voluntary sector services, Living Better supports those who may be isolated, vulnerable, lonely or struggling for any reason, by providing tailored support.
